A leading national physician services organization is recruiting a full-time Post-Acute Care Physician to serve a skilled nursing facility (SNF) panel in Jackson, Michigan. Practice Highlights

  • Attending physician services for a SNF resident panel of typically 75–125 patients
  • Full-time schedule of 4–5 days per week; on-call coverage included
  • Clinical scope includes admission H&Ps (within 48–72 hours), monthly recertifications, and significant-change-of-condition visits
  • Chronic disease management across CHF, COPD, diabetes, dementia, wound care, polypharmacy, and end-of-life/palliative care
  • Collaborative interdisciplinary team: NPs/PAs, nursing, PT/OT/SLP therapy, social work, and consultant pharmacy
  • Active participation in interdisciplinary care planning meetings, family conferences, and Medicare Part A clinical reviews
  • Documentation in PointClickCare or facility EMR, aligned with CMS Patient-Driven Payment Model (PDPM) standards

Qualifications

  • Board certification or board eligibility in Internal Medicine, Family Medicine, or Geriatrics required
  • Completed residency in Internal Medicine, Family Medicine, or Geriatrics; fellowship in Geriatrics a plus
  • Comfort managing the full post-acute spectrum: CHF, COPD, diabetes, dementia, palliative needs, and complex polypharmacy
  • Prior experience or genuine interest in long-term care, sub-acute rehabilitation, or skilled nursing strongly preferred
  • Active Michigan medical license or eligibility to obtain; active DEA registration where applicable
