This is a value-based practice model built around smaller patient panels, extended visit times, and a full clinical support team, where success is measured by patient health outcomes rather than visit volume. You will work alongside care coordinators, behavioral health specialists, and clinical pharmacists in a Medicare-focused clinic serving a rapidly expanding population of retirees on Florida's Treasure Coast.</p>
