Qualifications

  • Board certification (ABIM, ABFM, or ABIM Geriatric Medicine) in Internal Medicine, Family Medicine, or Geriatrics required
  • Prior SNF or long-term care clinical experience required
  • CMD (Certified Medical Director, AMDA) credential preferred; candidates willing to obtain within 24 months of hire will be considered
  • Demonstrated clinical leadership experience, including committee participation and regulatory familiarity (CMS, state survey processes)
  • Active Arizona medical license or eligibility to obtain; active DEA registration where applicable
