<p>Team Florida is recruiting a Skilled Nursing Facility Physician to lead post-acute care for senior patients across multiple partner facilities in the Tampa Bay market. This is a value-based role built around smaller patient panels, extended bedside time, and a full interdisciplinary team—success is measured by healing, stability, and safe transitions home rather than volume. You will round at a focused set of SNF partner buildings, working directly with nursing staff, therapists, social workers, and care coordination teams to keep residents healthy and out of the hospital. This is a pivotal position as Team Florida builds a stronger, more connected care model for seniors throughout Tampa Bay.</p>

<h3>Qualifications</h3> <ul> <li>MD or DO degree from an accredited medical school</li> <li>Board certification or board eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine</li> <li>Active and unrestricted Florida medical license</li> <li>Active DEA registration</li> <li>Current BLS certification</li> <li>Experience caring for geriatric or medically complex patients in a skilled nursing or post-acute setting preferred</li> <li>Comfort working within a value-based, team-based care model focused on outcomes rather than visit volume</li> <li>Strong communication and coordination skills across facility staff, specialists, and family members</li> <li>Willingness to travel between partner facilities within the Tampa Bay market</li> </ul>
