This is a value-based practice model with a focused patient panel, allowing you to spend meaningful time with each senior and work alongside physicians, care managers, medical assistants, behavioral health specialists, and social workers. You will manage chronic disease, lead preventive care, and coordinate the full spectrum of each patient's health — shaping senior care delivery in the Bayway community.</p>
