A leading national physician services organization is recruiting a full-time Post-Acute Care Primary Care Physician for a skilled nursing facility (SNF) setting in Springfield, Massachusetts. This is a W-2 employed attending role managing a defined resident panel — no hospital rounding, no ED call — with a clear leadership pathway into Facility Medical Director, Regional Medical Director, or CMO roles for physicians who want to grow beyond the bedside.

Practice Highlights

  • Attending physician coverage for a SNF resident panel of approximately 75–125 patients
  • Longitudinal primary care management for both long-term residents and short-stay post-acute rehabilitation patients
  • Clinical responsibilities include admission history and physicals, monthly recertifications, change-of-condition visits, and care plan participation
  • Collaborative interdisciplinary team (IDT) model: NPs/PAs, nursing, therapy, and social work integrated into daily care coordination
  • Coordination with specialist consultants and patient families as part of a structured care model
  • Full-time schedule with predictable, facility-based workflow — no overnight call or emergency department coverage
  • Administrative, billing, and credentialing functions handled centrally, allowing focus on clinical care

Qualifications

  • Board certification or board eligibility in Family Medicine or Internal Medicine
  • Active Massachusetts medical license or eligibility to obtain; DEA registration where applicable
  • Clinical comfort managing geriatric patients with multimorbidity, dementia, and end-of-life care needs
  • Prior experience in long-term care, primary care, or hospital medicine preferred
