Practice Highlights
- Attending physician coverage for a SNF resident panel of approximately 75–125 patients
- Longitudinal primary care management for both long-term residents and short-stay post-acute rehabilitation patients
- Clinical responsibilities include admission history and physicals, monthly recertifications, change-of-condition visits, and care plan participation
- Collaborative interdisciplinary team (IDT) model: NPs/PAs, nursing, therapy, and social work integrated into daily care coordination
- Coordination with specialist consultants and patient families as part of a structured care model
- Full-time schedule with predictable, facility-based workflow — no overnight call or emergency department coverage
- Administrative, billing, and credentialing functions handled centrally, allowing focus on clinical care
Qualifications
- Board certification or board eligibility in Family Medicine or Internal Medicine
- Active Massachusetts medical license or eligibility to obtain; DEA registration where applicable
- Clinical comfort managing geriatric patients with multimorbidity, dementia, and end-of-life care needs
- Prior experience in long-term care, primary care, or hospital medicine preferred
