<p>A leading national physician services organization is recruiting a full-time Post-Acute Care Physician to manage a skilled nursing facility (SNF) resident panel in Houston, TX. This is a pure clinical role with no call obligations, robust administrative infrastructure, and a clearly defined leadership pathway — from Facility Medical Director through Regional Medical Director to CMO — for physicians who want to grow beyond the bedside.</p>

<h3>Practice Highlights</h3> <ul> <li>Attending physician services for a SNF resident panel of approximately 75–125 patients</li> <li>Full-time schedule of 4–5 clinical days per week; no overnight or weekend call</li> <li>Clinical responsibilities include admission H&amp;Ps (within 48–72 hours), monthly recertifications, and significant-change-of-condition visits</li> <li>Manages the full post-acute and long-term care spectrum: chronic disease management, post-acute recovery, wound care, dementia care, complex polypharmacy, and end-of-life/palliative care</li> <li>Leads and collaborates with an interdisciplinary team including NPs/PAs, nursing, physical therapy, occupational therapy, speech-language pathology, social work, and consultant pharmacy</li> <li>Active participation in interdisciplinary care planning meetings, family conferences, and Medicare Part A clinical reviews</li> <li>Documentation in PointClickCare or facility EMR, following CMS Patient-Driven Payment Model (PDPM) standards</li> </ul>

<h3>Qualifications</h3> <ul> <li>Board certification or board eligibility in Internal Medicine, Family Medicine, or Geriatrics required</li> <li>Clinical comfort managing CHF, COPD, diabetes, dementia, palliative care needs, and complex polypharmacy in a post-acute population</li> <li>Prior experience or strong interest in long-term care, sub-acute rehabilitation, or skilled nursing facility medicine strongly preferred</li> <li>Active Texas medical license or eligibility to obtain; active DEA registration where applicable</li> </ul>
