Practice Highlights
- Attending physician services for a SNF resident panel of approximately 75–125 patients
- Full-time schedule: 4–5 days per week; on-call responsibilities included
- Clinical scope includes admission H&Ps (within 48–72 hours), monthly recertifications, and significant-change-of-condition visits
- Disease management focus: CHF, COPD, diabetes, dementia, wound care, polypharmacy, and end-of-life/palliative care
- Collaborative interdisciplinary team model: NPs/PAs, nursing, PT/OT/SLP therapy, social work, and consultant pharmacy
- Active participation in interdisciplinary care planning meetings, family conferences, and Medicare Part A clinical reviews
- Documentation in PointClickCare or facility EMR, aligned with CMS Patient-Driven Payment Model (PDPM) standards
Qualifications
- Board certification or board eligibility in Internal Medicine, Family Medicine, or Geriatrics required
- Clinical comfort managing CHF, COPD, diabetes, dementia, palliative care needs, and complex polypharmacy
- Prior experience or strong interest in long-term care, sub-acute rehabilitation, or skilled nursing settings strongly preferred
- Active Texas medical license or eligibility to obtain; active DEA registration where applicable
