<h3>Practice Highlights</h3> <ul> <li>Home health setting — RN travels to patients' homes in the Clermont, FL area to deliver and direct skilled nursing care per physician orders</li> <li>Conducts comprehensive nursing evaluation visits including full OASIS assessments and medication reconciliation</li> <li>Develops, initiates, implements, and revises individualized plans of care in collaboration with physicians and the broader care team</li> <li>Supervises home health aides and LPN/LVNs; assigns tasks and provides required supervisory visits in accordance with state and federal regulations</li> <li>Coordinates total plan of care across physicians, agency personnel, patients, families, and community resources to ensure continuity</li> <li>Communicates patient care issues, visit assignments, schedule changes, orders, OASIS data sets, and coding requests with appropriate agency staff in a timely manner</li> <li>Participates in on-call and weekend rotation as needed to meet patient needs</li> <li>Actively contributes to survey readiness, performance improvement initiatives, reduction of unnecessary hospitalizations, and patient safety best practices</li> <li>Serves as preceptor to new agency personnel, staff, and students</li> <li>Participates in the agency's utilization management model</li> <li>Documentation synced per organizational policy; visit documentation completed per payer requirements</li> </ul>

<h3>Qualifications</h3> <ul> <li>Current, active, and unrestricted RN licensure in the state of Florida</li> <li>Current CPR certification, or ability to obtain within 90 days of hire</li> <li>Valid driver's license, personal vehicle insurance, and access to a dependable vehicle or public transportation (travel to patient homes required)</li> <li>Ability to function effectively across diverse home environments regardless of patient age, background, disability status, or financial condition</li> <li>1+ years of home health experience preferred</li> <li>Demonstrated ability to work independently with strong communication, writing, and organizational skills preferred</li> <li>Must pass a pre-employment drug screening</li> </ul>
