<h3>Practice Highlights</h3> <ul> <li>Field-based home health practice serving patients in their private residences across The Villages, FL catchment area</li> <li>Full episode ownership: initiate, develop, implement, and revise individualized plans of care in collaboration with the ordering physician and interdisciplinary team</li> <li>Comprehensive OASIS assessments — including medication reconciliation — completed accurately and submitted per LHC policy timelines</li> <li>Supervisory responsibility over home health aides and LPN/LVNs, including task assignment per state and federal regulations and required supervisory visits</li> <li>Documentation and visit syncing conducted per payer requirements and agency policy; role requires timely electronic charting in the field</li> <li>Active participation in on-call and weekend rotation as needed to meet patient needs</li> <li>Involvement in survey readiness, performance improvement initiatives, and utilization management to reduce avoidable hospitalizations and improve patient safety outcomes</li> <li>Preceptorship and orientation support for new agency staff and students — a meaningful teaching component for experienced RNs</li> <li>Coordination hub role: communicates OASIS data sets, coding requests, schedule changes, and clinical updates across physicians, agency staff, patients, and families</li> <li>Part of a nationally scaled post-acute care network (LHC Group / Optum) with structured clinical support and development pathways</li> </ul>

<h3>Qualifications</h3> <ul> <li>Current, active, and unrestricted RN licensure in the state of Florida (required)</li> <li>Current valid driver's license, personal vehicle insurance, and access to a reliable personal vehicle (required — field-based role)</li> <li>Current CPR certification, or ability to obtain within 90 days of hire</li> <li>One or more years of home health experience preferred; OASIS competency and familiarity with home health eligibility and medical necessity criteria is a strong asset</li> <li>Demonstrated ability to work independently in unstructured field environments across a diverse patient population regardless of age, diagnosis, or socioeconomic background</li> <li>Strong written and verbal communication skills; solid organizational and time-management capabilities essential for managing a visit-based caseload</li> <li>Must pass a pre-employment drug screen</li> </ul>
