<h3>Practice Highlights</h3> <ul> <li>Field-based home health nursing — care delivered directly in patients' homes across the Vero Beach, FL service area</li> <li>Initiates, develops, implements, and revises individualized plans of care in collaboration with physicians and the broader interdisciplinary team</li> <li>Completes comprehensive OASIS assessments, including medication reconciliation, accurately and within required timeframes</li> <li>Performs initial and comprehensive nursing evaluation visits; determines home health eligibility and medical necessity in accordance with state guidelines</li> <li>Supervises home health aides and LPN/LVNs, provides instruction, and assigns tasks per state and federal regulations; conducts required supervisory visits</li> <li>Coordinates the total plan of care with physicians, agency personnel, patients, families, and community resources to maintain continuity of care</li> <li>Documents patient visits per policy and payer requirements; syncs documentation in accordance with agency policy</li> <li>Communicates patient care issues, visit assignments, schedule changes, physician orders, OASIS data sets, and coding requests with appropriate agency staff in a timely manner</li> <li>Participates in on-call and weekend rotation as needed to meet patient needs</li> <li>Actively engages in survey readiness, performance improvement initiatives, utilization management, and efforts to reduce unnecessary hospitalizations and improve patient safety</li> <li>Serves as a preceptor for new agency personnel and students; assists with orientation of incoming staff</li> </ul>

<h3>Qualifications</h3> <ul> <li>Current, active, and unrestricted RN licensure in the state of Florida</li> <li>Minimum 1 year of home health nursing experience required</li> <li>Current CPR certification</li> <li>Valid driver's license, personal vehicle insurance, and access to a dependable vehicle or reliable public transportation — field travel is inherent to this role</li> <li>Ability to function effectively across diverse home environments regardless of patient age, background, disability status, or financial condition</li> <li>Strong independent practice skills; solid written and verbal communication and organizational ability preferred</li> <li>Candidates must pass a pre-employment drug test</li> </ul>
