<h3>Practice Highlights</h3> <ul> <li>Home health setting — all patient care delivered in the patient's home environment across a range of ages, acuity levels, and socioeconomic backgrounds</li> <li>Full plan-of-care ownership: initiate, develop, implement, and revise individualized care plans in collaboration with the ordering physician and interdisciplinary team</li> <li>Comprehensive OASIS assessments completed accurately and on time, including medication reconciliation, eligibility determination, and medical necessity evaluation</li> <li>Documentation synced per organizational policy; visit documentation completed per payer requirements</li> <li>Supervisory responsibility over home health aides and licensed practical/vocational nurses, including task assignment in accordance with state and federal regulations and required supervisory visits</li> <li>Active coordination with physicians, agency staff, patients, families, and community resources to maintain continuity of care — including timely communication of orders, OASIS data sets, coding requests, and schedule changes</li> <li>Participation in on-call and weekend rotation as needed to meet patient needs</li> <li>Involvement in survey readiness, performance improvement initiatives, utilization management, and efforts to reduce preventable hospitalizations and improve patient safety outcomes</li> <li>Preceptor and orientation role for new agency personnel and students</li> <li>Operates within infection control standards and scope-of-practice guidelines consistent with state and federal regulations</li> </ul>

<h3>Qualifications</h3> <ul> <li>Current, active, and unrestricted Registered Nurse (RN) licensure in the state of Florida</li> <li>Minimum 1 year of home health nursing experience required</li> <li>Proficiency with OASIS comprehensive assessments and home health documentation requirements</li> <li>Current CPR certification required</li> <li>Valid driver's license, personal vehicle insurance, and access to a dependable vehicle or reliable public transportation — field-based role requiring travel to patient homes</li> <li>Ability to function effectively across all home environments regardless of patient age, background, disability status, or financial condition</li> <li>Preferred: strong independent practice skills; solid written, verbal, and organizational communication abilities</li> <li>Pre-employment drug screening required</li> </ul>
