<h3>Practice Highlights</h3> <ul> <li>Fully field-based, home-visit model — all patient encounters conducted in members' homes across Hendry and Lee counties</li> <li>Conduct comprehensive annual in-home health assessments for Medicare Advantage and other plan members</li> <li>Assessment scope includes past medical history review, medication reconciliation, vital signs, physical examination, evidence-based screenings, and point-of-care testing as appropriate</li> <li>Identify and document diagnoses to support care management and treatment planning; communicate findings directly to members' primary care providers to close gaps in care</li> <li>Educate members on disease processes, medication adherence, and self-management strategies</li> <li>Address social determinants of health and coordinate referrals to key support services</li> <li>Recognize and appropriately intervene in urgent or emergent clinical situations encountered during home visits</li> <li>Non-prescribing role — clinical focus is on assessment, education, gap closure, and PCP collaboration within an interdisciplinary care model</li> <li>Proficiency with electronic medical records required; technology-enabled documentation platform used in the field</li> <li>Role requires reliable personal transportation, a valid driver's license, and the ability to transport equipment up to 30 pounds and navigate stairs</li> </ul>

<h3>Qualifications</h3> <ul> <li>For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care</li> <li>For PAs: Current NCCPA certification and active state licensure</li> <li>Active, unrestricted Florida licensure in good standing (or ability to obtain prior to start date); no current disciplinary actions or practice limitations</li> <li>Active, unrestricted driver's license and access to reliable personal transportation for home visits</li> <li>NPs/PAs practicing in a state permitting independent practice must obtain licensing board approval if required — new hires eligible at hire must apply within one month of start; those not yet eligible must begin working toward eligibility within one month and apply within three months of becoming eligible</li> <li>Preferred: one or more years of clinical experience in family medicine, geriatrics, or home health settings</li> <li>Preferred: demonstrated communication skills with geriatric or Medicare populations and proficiency with EMR platforms</li> <li>Physical requirements include the ability to sit, stand, and kneel as needed to perform assessments in varied home environments</li> <li>Pre-employment drug screening required</li> </ul>
