<h3>Practice Highlights</h3> <ul> <li>Pediatric private duty nursing — one-on-one direct care for acutely ill and chronically ill pediatric patients in their home environments</li> <li>Care delivered in accordance with physician-prescribed plans of care and in coordination with the broader interdisciplinary health care team</li> <li>Practice governed by Florida state nursing law and applicable infection control standards</li> <li>Documentation completed in an electronic medical record with electronic visit verification (EVV) program</li> <li>Active participation in care coordination: timely reporting of patient status changes, attendance at patient case conferences, and communication with patients, families, physicians, clinicians, agency management, case coordinators, and community case managers</li> <li>Work performed under the direct supervision of a Registered Nurse and/or designated supervisor</li> <li>Service area covers Mount Dora and surrounding communities in the greater Lake County region</li> <li>Part of Mederi Private Care, a division of LHC Group and a member of the Optum/UnitedHealth Group family of businesses — one of the largest post-acute care networks in the country</li> </ul>

<h3>Qualifications</h3> <ul> <li>Current, active, and unrestricted Licensed Practical Nurse (LPN) licensure in the state of Florida</li> <li>Current CPR certification</li> <li>Valid driver's license with current vehicle insurance and access to a dependable vehicle, or reliable access to public transportation</li> <li>Ability to deliver care across diverse home environments regardless of patient age, background, or financial condition</li> <li>Home care or private duty nursing experience preferred</li> <li>Demonstrated ability to work independently, manage multiple tasks simultaneously, and communicate effectively in writing and verbally</li> <li>Candidates must pass a pre-employment drug screening</li> </ul>
