<h3>Practice Highlights</h3> <ul> <li>Monday–Friday schedule, 8:00 AM–5:00 PM; no nights, no weekends, no call</li> <li>Embedded within a clinic serving Medicare-age adults as part of a network with over 16,000 physician offices and 22,000+ clinicians nationally</li> <li>Caseload focused on high-risk patients with complex chronic conditions including COPD/asthma, diabetes, congestive heart failure (CHF), and ischemic heart disease (IHD)</li> <li>Longitudinal care management role: conducts health assessments, medication reconciliation, motivational interviewing, and self-management goal setting across the care continuum</li> <li>Active role in care transitions — supports patients moving from inpatient to outpatient settings, including post-discharge coordination of prescriptions, DME, transportation, and follow-up appointments</li> <li>Participates in daily huddles and Patient Care Coordination (PCC) meetings; prepares weekly reports for care team review</li> <li>Collaborates with a multidisciplinary team: physicians, nurses, pharmacists, laboratory technologists, social workers, and patient educators</li> <li>Documents in an electronic medical record system; proficient EMR use is a core expectation of the role</li> <li>Self-directed work environment with latitude to identify at-risk patients and drive individualized care plans in partnership with providers</li> <li>May perform clinical tasks within RN scope of practice as needed</li> </ul>
<h3>Qualifications</h3> <ul> <li>Bachelor of Science in Nursing (BSN) required; Associate Degree in Nursing (ADN) accepted with 4 or more years of relevant experience</li> <li>Active, unrestricted Registered Nurse license in the state of Florida</li> <li>Current BLS certification required</li> <li>Minimum 2 years of experience in a physician office, clinical, or hospital setting</li> <li>Demonstrated knowledge of chronic disease management, particularly COPD/asthma, diabetes, CHF, and ischemic heart disease</li> <li>Proficient with electronic medical records; ability to document accurately and efficiently</li> <li>Tuberculosis screening required; documented immunity or vaccination required for Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis prior to start</li> <li>Preferred: experience with patient education, motivational interviewing, and self-management goal-setting techniques</li> <li>Preferred: strong verbal and written communication skills with demonstrated ability to work productively within multidisciplinary teams</li> </ul>
