<h3>Practice Highlights</h3> <ul> <li>Home health setting — RN provides direct nursing care to patients in their place of residence as prescribed by the treating physician</li> <li>Responsible for initiating, developing, implementing, and revising individualized plans of care in collaboration with physicians and the broader interdisciplinary team</li> <li>Completes comprehensive OASIS assessments, including medication reconciliation, accurately and within required timeframes</li> <li>Supervises home health aides and licensed practical/vocational nurses; assigns tasks and provides instruction in accordance with state and federal regulations</li> <li>Conducts initial and comprehensive nursing evaluation visits; determines home health eligibility and medical necessity; establishes primary focus of care within state guidelines</li> <li>Coordinates total plan of care across physicians, agency personnel, patients, families, and community resources to ensure continuity</li> <li>Documents patient visits per policy and payer requirements; syncs documentation per organizational policy</li> <li>Communicates patient care issues, visit assignments, schedule changes, orders, OASIS data sets, coding requests, and clinician coordination to appropriate agency staff in a timely manner</li> <li>Participates in on-call and weekend rotation as needed to meet patient needs</li> <li>Actively participates in survey readiness, performance improvement initiatives, and utilization management; works to reduce unnecessary hospitalizations and improve patient safety</li> <li>Serves as a preceptor for new agency personnel, staff, and students</li> </ul>

<h3>Qualifications</h3> <ul> <li>Current, active, and unrestricted RN licensure in the state of Florida</li> <li>Minimum 1 year of home health nursing experience required</li> <li>Current CPR certification required</li> <li>Valid driver's license, personal vehicle insurance, and access to a dependable vehicle or reliable public transportation — field-based role requiring travel to patient homes</li> <li>Ability to function effectively across diverse patient populations regardless of age, race, creed, color, sex, disability, or financial condition</li> <li>Strong independent practice skills; solid written and verbal communication and organizational abilities preferred</li> <li>Candidates must pass a pre-employment drug screening</li> </ul>
