<h3>Practice Highlights</h3> <ul> <li>Multi-county market footprint covering Pasco, Polk, Hernando, and Citrus counties — field travel is expected and integral to the role</li> <li>Serves the highest-risk health plan beneficiaries through a longitudinal, integrated care delivery model in home and community settings</li> <li>Interdisciplinary team environment: the CSM mentors and coaches a panel of Advanced Practice Clinicians (APCs) across the market</li> <li>Conducts scheduled Ride Along visits with APCs in accordance with senior leadership guidelines to ensure clinical quality and program fidelity</li> <li>Responsible for full workforce management functions: hiring, onboarding, training, scheduling, staffing plans, status changes, terminations, and PTO management via GSS/myHR systems</li> <li>Leads monthly team meetings to communicate company goals, address field concerns, and relay feedback to senior leadership</li> <li>Conducts annual performance reviews and implements coaching and corrective action plans aligned to business standards</li> <li>Develops and presents market strategy to senior leadership during formal Business Reviews</li> <li>Ensures compliance with enterprise, departmental, HIPAA, and regulatory requirements, including protection of PHI and intellectual property</li> <li>Collaborates regularly with supporting departments to optimize operational efficiency, scheduling, and staffing assignments</li> <li>Applies workforce forecasting models to manage a rapidly changing staffing environment and make data-driven decisions</li> <li>Identifies high-potential staff and creates pathways for professional growth and career advancement to reduce turnover</li> <li>Program operates within a managed care framework with attention to HEDIS and STAR quality measures</li> </ul>

<h3>Qualifications</h3> <ul> <li>For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care</li> <li>For PAs: Current NCCPA certification and active state licensure</li> <li>Active, unrestricted Florida licensure (or ability to obtain by start date) with no current disciplinary actions or practice limitations</li> <li>Minimum 5 years of clinical practice experience in a clinical setting</li> <li>Active, unrestricted driver's license and access to reliable personal transportation for field visits across the four-county market</li> <li>Physical ability to navigate varied environments and position oneself as needed to perform job duties</li> <li>NPs/PAs practicing in states permitting independent practice must obtain licensing board approval (if required) within 1 month of hire, or begin working toward eligibility within 1 month and apply within 3 months of becoming eligible</li> <li>Preferred: 2 or more years of management or leadership experience</li> <li>Preferred: Prior experience in a home-based care setting and/or managed care environment</li> <li>Preferred: Working knowledge of HEDIS and STAR quality measures</li> <li>Preferred: Proficiency with electronic medical records and clinical technology platforms</li> </ul>
