<h3>Practice Highlights</h3> <ul> <li>Provides direct skilled nursing care to patients in their home environment, operating under physician-prescribed plans of care and in compliance with applicable state and federal regulations</li> <li>Conducts initial and comprehensive nursing evaluation visits, including full OASIS assessments and medication reconciliation, completed accurately and on time</li> <li>Develops, implements, and revises individualized plans of care in collaboration with physicians and the broader interdisciplinary care team</li> <li>Supervises home health aides and LPN/LVNs, assigns tasks per state and federal regulatory requirements, and provides required supervisory visits</li> <li>Coordinates care continuity by communicating with physicians, agency staff, patients, families, and community resources — including visit assignments, schedule changes, orders, OASIS data sets, and coding requests</li> <li>Documents patient visits per policy and payer requirements; syncs documentation timely per agency policy</li> <li>Participates in on-call and weekend rotation as needed to meet patient needs</li> <li>Actively engages in survey readiness, performance improvement initiatives, and utilization management to reduce unnecessary hospitalizations and improve patient safety outcomes</li> <li>Serves as a preceptor for new agency personnel and students; assists with orientation of incoming staff</li> <li>Weekly visit volume varies based on individual productivity targets and the point values assigned to each visit type</li> </ul>

<h3>Qualifications</h3> <ul> <li>Current, active, and unrestricted RN licensure in the state of Florida</li> <li>Valid driver's license, personal vehicle insurance, and reliable transportation (personal vehicle or public transit)</li> <li>Current CPR certification, or ability to obtain within 90 days of hire</li> <li>One or more years of home health nursing experience preferred</li> <li>Demonstrated ability to work independently and manage a patient caseload with minimal direct supervision</li> <li>Strong written and verbal communication skills; solid organizational and documentation abilities</li> </ul>
