<p>A hospice-focused home health organization operating under a nationally recognized post-acute care network is recruiting a Per Diem (PRN) Registered Nurse Case Manager for its hospice division in Melbourne, Florida. This field-based role places an experienced RN at the center of end-of-life care coordination — owning the full Plan of Care for each patient from initial eligibility evaluation through ongoing interdisciplinary collaboration. <h3>Practice Highlights</h3> <ul> <li>Per diem RN Case Manager role with full nursing accountability for each patient's hospice Plan of Care</li> <li>Conducts initial nursing eligibility evaluations within 48 hours of referral</li> <li>Performs comprehensive assessment of patient and family physical, psychosocial, and environmental needs; reassesses no less than every 15 days</li> <li>Initiates, coordinates, and maintains the individualized Plan of Care with accurate, relevant clinical documentation throughout the episode</li> <li>Documents patient problems, goals, interventions, and patient/family responses to hospice care</li> <li>Collaborates daily with patients, families, attending physicians, and all members of the Interdisciplinary Group (IDG)</li> <li>Field-based, community-facing role requiring reliable personal transportation or access to public transit</li> <li>Operates within a nationally scaled post-acute care network with established hospice protocols and quality standards</li> </ul>

<h3>Qualifications</h3> <ul> <li>Current, unrestricted RN licensure in the state of Florida</li> <li>Minimum 1 year of clinical nursing experience</li> <li>Valid driver's license and current vehicle insurance with access to a dependable vehicle, or reliable access to public transportation</li> <li>Current CPR certification preferred, or ability to obtain within 90 days of hire</li> </ul>
