A full-time Post-Acute Care Physician opportunity is available in Dallas, TX, serving residents in a skilled nursing facility (SNF) setting. This employed position offers a structured, team-based care environment with a defined patient panel and strong organizational support.

Practice Highlights

  • Full-time schedule, 4–5 days per week, with on-call responsibilities
  • Attending physician services for a SNF resident panel of approximately 75–125 patients
  • Conduct admission H&Ps within 48–72 hours, monthly recertifications, and significant-change-of-condition visits
  • Manage chronic disease, post-acute recovery, wound care, dementia care, polypharmacy, and end-of-life care
  • Lead and collaborate with NPs/PAs, nursing, therapy (PT/OT/SLP), social work, and consultant pharmacy
  • Participate in interdisciplinary care planning, family conferences, and Medicare Part A clinical reviews
  • Document in PointClickCare or facility EMR per CMS PDPM standards

Qualifications

  • Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatrics
  • Active Texas state license (or eligibility) and DEA registration where applicable
  • Clinical comfort managing CHF, COPD, diabetes, dementia, palliative needs, and complex polypharmacy
  • Prior experience or strong interest in long-term care, sub-acute rehabilitation, or skilled nursing preferred
