This field-based Advanced Practice Clinician role — open to Nurse Practitioners and Physician Assistants — covers a nine-state Southeast territory (Alabama, Arkansas, Florida, Georgia, Kentucky, Louisiana, Mississippi, North Carolina, and South Carolina) and is based out of Orlando, FL, though candidates may live anywhere within the coverage footprint. The position sits within an integrated home-and-community care model focused on annual in-home health assessments for Medicare Advantage members, with a strong emphasis on preventive care, care-gap closure, and PCP collaboration. The most compelling differentiator: no call, no weekends, no holidays, fully covered travel expenses, and a compensation package that includes a $30,000 sign-on bonus or up to $40,000 in student loan repayment.

Perks and Benefits

Base salary range of $109,500–$164,000, with total cash compensation (base plus bonus) determined by local labor market, education, and experience — and structured to grow with productivity and performance

$30,000 sign-on bonus or up to $40,000 in student loan repayment for external candidates and eligible internal candidates

All travel expenses fully covered — no out-of-pocket cost for the approximately 85% travel requirement

Flexible scheduling across a 7-day window (8 a.m.–8 p.m.) with no on-call, no required weekends, and no required holidays

Incentive and recognition programs in addition to base compensation

Equity stock purchase plan and 401(k) contribution program

Comprehensive benefits package including health, dental, and vision (all benefits subject to eligibility requirements)

Drug-free workplace; candidates must pass a pre-employment drug screen

Practice Highlights

Conduct comprehensive annual in-home health assessments for Medicare Advantage and other health plan members across a nine-state Southeast territory

Assessment scope includes: past medical history review, medication reconciliation, vital signs, physical examination, evidence-based screenings, and point-of-care testing as appropriate

Identify and document diagnoses to support care management and treatment planning; communicate findings directly to members' primary care providers to close gaps in care

Non-prescribing role — clinical focus is on assessment, education, gap closure, and PCP coordination rather than ongoing treatment management

Address social determinants of health and connect members to appropriate referral and support services

Recognize and respond to urgent and emergent clinical situations encountered during home visits

Educate members on disease processes, medication adherence, and self-management strategies

Field-based, autonomous practice model within an interdisciplinary care environment; providers flex their own schedules between 8 a.m. and 8 p.m. across the seven-day week

Travel approximately 85% of the time across the assigned state(s); must be able to navigate varied home environments including stairs and confined spaces, and transport assessment equipment up to 30 pounds

Electronic medical records used throughout the program; proficiency with EMR and clinical technology is expected

Qualifications

For Nurse Practitioners: active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

For Physician Assistants: current NCCPA certification and active state licensure

Active, unrestricted licensure in the state(s) of practice — or demonstrated ability to obtain licensure by start date — with no current disciplinary actions or practice limitations

Active, unrestricted driver's license and access to reliable personal transportation for home visits

NPs/PAs in states permitting independent practice must obtain licensing board approval (if required); new hires eligible at hire must apply within one month of start; those not yet eligible must begin working toward eligibility within one month and apply within three months of becoming eligible

One or more years of clinical experience preferred, with family medicine, geriatrics, or home health backgrounds particularly well suited to this role

Proven communication skills with geriatric and Medicare populations preferred

Physical ability to sit, stand, and kneel as needed to perform in-home assessments, navigate stairs, and transport equipment up to 30 pounds

About the Community

Because this role allows clinicians to live anywhere within the nine-state Southeast footprint, lifestyle flexibility is a genuine feature of the position — whether that means the Gulf Coast beaches of Florida or Alabama, the Blue Ridge foothills of North Carolina or Georgia, the vibrant cultural corridors of New Orleans or Charlotte, or the lower cost of living found across much of Arkansas, Mississippi, and Kentucky. The Southeast offers a wide range of climates, outdoor recreation, and urban-to-rural living options, making it straightforward to anchor a home base that fits personal and family priorities while maintaining a fully mobile clinical practice.
