<p>A per-diem (PRN) Medical Social Worker opportunity is open with a post-acute home health organization serving the Lake City, Florida region. Working under physician direction and alongside an Interdisciplinary Group, you will deliver medical social services that address the social and emotional dimensions of patient health — helping patients and families develop coping strategies, navigate resources, and move toward better outcomes. This is a flexible, as-needed engagement well suited to an MSW-level clinician who wants meaningful community-based work without a fixed full-time schedule.</p>

<h3>Qualifications</h3> <ul> <li>Master's Degree in Social Work from a Council on Social Work Education (CSWE)-accredited program — required</li> <li>Current, valid driver's license with personal vehicle insurance and access to reliable transportation — required</li> <li>Licensed Social Worker (LSW or LCSW) in the state of Florida — preferred</li> <li>Current CPR certification, or ability to obtain within 90 days of hire — preferred</li> <li>One or more years of social work experience in a healthcare setting — preferred</li> <li>Must pass a pre-employment drug screening</li> </ul>
