<h3>Practice Highlights</h3> <ul> <li>Full-time schedule: Monday–Friday, 8:30 AM–5:00 PM Eastern; specific schedule confirmed by manager at hire</li> <li>Hybrid field/home-office model — work from a dedicated home workspace when not conducting field visits; local travel approximately 25% of the time, up to 100 miles round trip</li> <li>Primary population: Medicaid members across Miami, FL and surrounding Miami-Dade County communities</li> <li>Core focus on social determinants of health: transportation, housing stability, food access, financial resources, and psychosocial support</li> <li>Member engagement conducted both face-to-face in the field and telephonically, using Motivational Interviewing techniques to support person-centered SMART goal development</li> <li>Responsibilities include individualized care plan development, referral initiation for social service programs (financial, psychosocial, community, and state-level), and post-discharge follow-up from hospital facilities</li> <li>Collaboration with patients, families, healthcare providers, and Peer Support specialists to ensure coordinated, quality care</li> <li>Documentation of all member assessments, care plans, and referrals maintained in the member's health record with strict adherence to PHI security requirements</li> <li>Accountable to utilization management benchmarks and quality outcome metrics</li> <li>Supported by a team-based managed care infrastructure with access to both company and community-based resources</li> </ul>

<h3>Qualifications</h3> <ul> <li>High School Diploma or GED required</li> <li>2+ years of field-based work experience required</li> <li>1+ year of demonstrated knowledge of community resources, culture, and values — specifically within Miami-Dade County communities</li> <li>Bilingual fluency in Spanish and English required</li> <li>Must reside within Miami-Dade County and within commutable distance to Miami and surrounding service areas</li> <li>Valid, unrestricted Florida Driver's License with state-required auto insurance and reliable personal transportation capable of local travel up to 100 miles round trip</li> <li>1+ year of computer proficiency including Microsoft Word, Excel, and Outlook; ability to maintain a secure, designated home workspace compliant with PHI protection standards</li> <li>Preferred: CHW accreditation; care management or managed care experience; knowledge of Medicaid and/or Medicare populations; background in behavioral health or substance use disorders</li> </ul>
