<h3>Practice Highlights</h3> <ul> <li>Home health setting — patients seen in their place of residence across the Lake City, FL service area</li> <li>Caseload spans functional status and mobility assessment, endurance and stamina evaluation, pain management, and complex medical condition management</li> <li>Clinician develops and executes individualized PT plans of care in collaboration with the supervising nurse and referring physician</li> <li>Performs therapeutic interventions aimed at pain relief, functional restoration, and maintenance of maximum independence</li> <li>Completes progress notes per visit and submits documentation in accordance with organizational policy; all patient evaluations and PT plans of care completed within state-specific timelines</li> <li>Reports evaluation outcomes, treatment goals, and projected frequency of care to the broader care team</li> <li>Coordinates with interdisciplinary team members — nurses, physicians, and family caregivers — on plan of care updates and patient education</li> <li>Observes and communicates changes in patient condition to supervising nurse and/or physician in real time</li> <li>Independent, community-based schedule requiring reliable personal transportation or access to public transit</li> <li>Part of a nationally scaled post-acute care network affiliated with Optum/UnitedHealth Group, with access to enterprise-level resources and infrastructure</li> </ul>

<h3>Qualifications</h3> <ul> <li>Current Physical Therapy licensure in the state of Florida (or eligibility to obtain)</li> <li>Current CPR certification required</li> <li>Valid driver's license and personal vehicle insurance, with access to a dependable vehicle or public transportation</li> <li>Ability to function effectively across diverse home environments regardless of patient age, background, disability status, or financial condition</li> <li>Preferred: demonstrated ability to manage multiple patient cases simultaneously with strong independent work ethic</li> <li>Preferred: strong written and verbal communication skills and high-level organizational capability</li> <li>Must pass a pre-employment drug screening</li> </ul>
