<p>This full-time, in-office Intake Coordinator role is based in Longwood, FL, supporting a home health agency that operates as part of a large national post-acute care network. The position sits at the center of the referral intake process — triaging incoming calls, entering referrals into the operating system, and coordinating clinical and demographic information between referral sources, central intake nurses, and branch offices. The schedule is a consistent Monday through Friday, 8:00 AM to 4:30 PM, with no evenings or weekends.</p>

<h3>Practice Highlights</h3> <ul> <li>Monday through Friday schedule, 8:00 AM to 4:30 PM, fully in-office — no remote or hybrid component</li> <li>Serves as the first point of contact for all incoming referral calls, triaging to the appropriate Central Intake team member or branch</li> <li>Collects and enters demographic and clinical referral data into the mainframe operating system for both central intake and branch referrals</li> <li>Coordinates directly with Central Intake Nurses on high-tech and nurse-specific referral calls, escalating appropriately</li> <li>Manages patient ping feed throughout the day in applicable markets, disseminating data to patient care managers, field clinicians, and sales representatives</li> <li>Communicates with physicians' offices, clients, and other referral sources to compile complete and accurate referral records</li> <li>Exercises independent judgment in obtaining information from callers when the appropriate staff member is unavailable</li> <li>Provides general agency service information to patients, families, and referral sources as needed</li> <li>Participates in ongoing departmental workshops, continuing education programs, and annual in-service training</li> <li>Operates within a national post-acute care network with established protocols and centralized intake infrastructure</li> </ul>

<h3>Qualifications</h3> <ul> <li>Minimum of one year of experience in office procedures</li> <li>Minimum of one year of insurance or medical office experience</li> <li>Ability to triage multi-line phone volume and relay clinical referral information accurately and efficiently</li> <li>Comfort with data entry into healthcare operating systems or mainframe platforms</li> <li>Must pass a pre-employment drug screening</li> </ul>
