<h3>Practice Highlights</h3> <ul> <li>Monday through Friday schedule, 8:00 a.m. to 4:30 p.m. — primarily office-based with occasional patient visits in the field</li> <li>Responsible for overall supervision and coordination of clinical services across an assigned home health team area</li> <li>Coordinates and supervises an interdisciplinary clinical team to ensure continuity and quality of care aligned with physician-prescribed plans of care</li> <li>Manages the full referral intake workflow: receives referrals, assigns appropriate clinicians, evaluates patient orders, and schedules start-of-care visits</li> <li>Oversees determination of home health benefits, medical necessity documentation, and ongoing insurance authorization and approvals</li> <li>Conducts daily review of clinical assessments and plans of care per assigned workflow; provides real-time consultation and recommendations to field clinicians</li> <li>Ensures individualized, patient-centered care that meets home health eligibility criteria and complies with all applicable state and federal regulations</li> <li>Retains the ability to provide direct clinical services within the RN scope of practice when needed, across a diverse patient population regardless of age, background, or financial status</li> <li>Part of a nationally scaled post-acute care network with hospital, physician, and family partnerships across the country</li> </ul>

<h3>Qualifications</h3> <ul> <li>Current, unrestricted RN licensure in the state of Florida (required)</li> <li>Current driver's license, personal vehicle insurance, and access to a reliable vehicle or public transportation (required)</li> <li>Current CPR certification, or ability to obtain within 90 days of hire (preferred)</li> <li>Prior home health or home care experience strongly preferred</li> <li>Demonstrated ability to supervise interdisciplinary clinical teams and manage care coordination workflows</li> </ul>
