<h3>Practice Highlights</h3> <ul> <li>Field-based role with 75–85% travel to contracted primary care provider offices across the assigned Florida market</li> <li>Reports to the Provider Relations Leader within a matrixed leadership structure; clinical activities directed by the WellMed Senior Medical Director</li> <li>Manages patient census across assigned practices with a focus on high-risk, high-cost cohorts — including Band 5 members requiring monthly in-person visits</li> <li>Monitors and actively influences key utilization metrics: ER visits, Admits/K, Readmits/K, SNF/ASC/hospital usage, and palliative care engagement</li> <li>Conducts weekly touchpoints with the medical director dyad partner to review admissions and determine escalation pathways</li> <li>Executes weekly "tuck-in calls" to high-risk members and facilitates timely post-discharge follow-up for transitions of care</li> <li>Supports contingency planning for high-risk diagnoses (heart failure, COPD), including advanced directive documentation and care bundle implementation</li> <li>Drives closure of HEDIS/STAR care gaps: medication adherence, preventive screenings, and quality measure documentation</li> <li>Facilitates Tier 1 Cardiology/Heart Failure clinic referrals and supports optimization of guideline-directed medical therapy (GDMT)</li> <li>Partners with PBM and Medical Director teams to analyze utilization and performance data, identify trends, and develop market-aligned action plans covering quality, cost, and risk adjustment coding</li> <li>Provides education and baseline support for risk adjustment documentation prior to coder SME engagement; assists practices in applying QRA strategies</li> <li>Produces scheduled and ad-hoc reports on key performance metrics for internal and provider-facing audiences</li> <li>Network context: 22,000+ primary care physicians, hospitalists, specialists, and advanced practice clinicians serving 900,000+ older adults — partnership with Optum established 2011</li> </ul>

<h3>Qualifications</h3> <ul> <li>Associate's Degree in Nursing required; Bachelor's degree in Business or Healthcare Administration preferred</li> <li>Current, unrestricted RN license in Florida required, or ability to obtain a compact license within 30 days of hire</li> <li>Minimum 4 years of combined experience in provider relations, network management, care management, and/or clinical operations</li> <li>Working knowledge of Medicare Advantage, HEDIS, STAR measures, CMS reimbursement models, and risk adjustment methodology</li> <li>Demonstrated proficiency in analytics, relationship-building, communication, and Microsoft Office suite</li> <li>CPC (Certified Professional Coder) certification preferred</li> <li>Experience in value-based care or population health management preferred</li> <li>Valid state-issued driver's license and ability to travel up to 75–85% of the time within the assigned market</li> <li>Ability to lift up to 25 lbs. and perform extended periods of sitting, standing, and driving</li> </ul>
