<h3>Practice Highlights</h3> <ul> <li>Home health setting — patients seen in their place of residence across the Gainesville, FL service area</li> <li>Caseload includes functional status and mobility assessments, endurance and stamina evaluation, pain management, and management of complex medical conditions</li> <li>Responsible for completing full PT evaluations and developing individualized plans of care within Florida state-specific timelines and guidelines</li> <li>Performs therapeutic interventions aimed at pain relief, functional restoration, and maintenance of maximum function using evidence-based, accepted standards of practice</li> <li>Collaborates directly with supervising nurses and referring physicians on plan-of-care development and patient progress reporting</li> <li>Documents a progress note for each patient visit and submits per organizational policy; reports evaluation outcomes, goals, and projected frequency of care</li> <li>Instructs patients, family members, and caregivers on functional limitations, home exercise programs, and progress toward established goals</li> <li>Serves a diverse patient population regardless of age, diagnosis, or socioeconomic background</li> <li>Part of a post-acute care network with hospital, physician, and family partnerships across the country</li> </ul>

<h3>Qualifications</h3> <ul> <li>Active Physical Therapy licensure in the state of Florida (required at time of hire)</li> <li>Graduate of an accredited Physical Therapy program (DPT or equivalent)</li> <li>Current CPR certification</li> <li>Valid driver's license with personal vehicle insurance and access to a reliable vehicle (or access to public transportation) — field-based travel is required</li> <li>Ability to practice independently across varied home environments and patient demographics</li> <li>Candidates must pass a pre-employment drug screening</li> </ul>
