<h3>Practice Highlights</h3> <ul> <li>Home health setting — all patient care delivered in the patient's place of residence across the Winter Haven, FL service area</li> <li>Responsible for assessment, evaluation, and treatment planning for patients with functional mobility, strength, and rehabilitation needs</li> <li>Applies diagnostic and prognostic functional ability testing to establish baseline and track progress</li> <li>Collaborates directly with the supervising nurse and referring physician to develop and refine the therapy plan of care</li> <li>Instructs patients, family members, caregivers, and interdisciplinary team members in home exercise programs and therapeutic techniques</li> <li>Documents and reports patient response to treatment and changes in condition to the supervising nurse and/or physician</li> <li>Weekend rotation required — schedule otherwise structured around PRN availability</li> <li>Practice governed by Florida state PT scope-of-practice statutes and coordinated within the broader interdisciplinary home health team</li> </ul>

<h3>Qualifications</h3> <ul> <li>Active Physical Therapy license in the state of Florida (state of residence)</li> <li>Current CPR certification required</li> <li>Valid driver's license, personal vehicle insurance, and access to a dependable vehicle for home visits throughout the service area</li> <li>Availability to participate in a weekend rotation</li> <li>Candidates must pass a pre-employment drug screening</li> </ul>
