<h3>Practice Highlights</h3> <ul> <li>Conduct comprehensive annual in-home health assessments for Medicare Advantage and other health plan members across Osceola and Orange Counties, FL</li> <li>Assessment scope includes: past medical history review, medication reconciliation, vital signs, physical examination, evidence-based screenings, and point-of-care testing as appropriate</li> <li>Identify and document diagnoses to support care management and treatment planning</li> <li>Communicate clinical findings directly to members' primary care providers to close gaps in care</li> <li>Recognize and respond to urgent and emergent clinical situations encountered in the home setting</li> <li>Educate members on disease processes, medication adherence, and self-management strategies</li> <li>Address social determinants of health and coordinate referrals to appropriate support services</li> <li>Non-prescribing role — focus is entirely on assessment, education, gap closure, and PCP collaboration</li> <li>Interdisciplinary care model integrating physical, mental, and social health needs</li> <li>Electronic medical records used throughout; technology proficiency is a working expectation</li> <li>Field-based role requiring reliable personal transportation; equipment transport up to 30 lbs and stair navigation are part of routine home visits</li> </ul>

<h3>Qualifications</h3> <ul> <li>For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care</li> <li>For PAs: Current NCCPA certification and active state licensure</li> <li>Active, unrestricted Florida licensure (or ability to obtain prior to start date) with no current disciplinary actions or practice limitations</li> <li>Active, unrestricted driver's license and access to reliable personal transportation for home visits</li> <li>Physical ability to navigate varied home environments, including sitting, standing, and kneeling as required to perform assessments</li> <li>NPs/PAs practicing in states permitting independent practice must obtain licensing board approval (if required) within one month of hire, or begin working toward eligibility within one month and apply within three months of becoming eligible</li> <li>Preferred: 1+ years of clinical experience in family medicine, geriatrics, or home health</li> <li>Preferred: Demonstrated communication skills with geriatric or Medicare populations</li> <li>Preferred: Proficiency with electronic medical records and clinical technology platforms</li> </ul>
