<h3>Practice Highlights</h3> <ul> <li>Care management role spanning both inpatient and outpatient settings — work is performed on-site and telephonically based on patient need</li> <li>Functions as care team leader in collaboration with the primary care physician and broader interdisciplinary care team</li> <li>Manages transitions of care for high-risk patients across acute care hospitals, skilled nursing facilities, custodial care, and ambulatory settings</li> <li>Facilitates discharge planning including follow-up appointments, home health referrals, social services coordination, and transportation arrangements</li> <li>Applies established disease management protocols and care management pathways approved by medical management to optimize clinical outcomes</li> <li>Maintains active communication with physicians, hospitalists, extended care facilities, disease management teams, patients, and families</li> <li>Utilizes health plan benefit data, utilization metrics, and care management reporting systems to inform and evaluate care decisions</li> <li>Addresses emerging patient care issues on initial visit and prioritizes needs in real time</li> <li>HMO/managed care environment with emphasis on appropriate resource utilization and quality outcomes</li> </ul>

<h3>Qualifications</h3> <ul> <li>High school diploma, GED, or equivalent required</li> <li>Valid Florida LPN or RN license required</li> <li><strong>LPN pathway:</strong> Certificate in Practical Nursing; IV certification required or must be obtained within 90 days of hire</li> <li><strong>RN pathway:</strong> Associate's or Bachelor's degree in Nursing Science required</li> <li>Basic Life Support (BLS) for Healthcare Providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross) required — must obtain within 90 days of hire if not currently certified</li> <li>One or more years of experience in an acute clinical care setting preferred</li> <li>Prior care management, utilization review, or discharge planning experience preferred</li> <li>HMO or managed care experience preferred</li> <li>Bilingual fluency in Spanish and English preferred</li> <li>Candidates must pass a pre-employment drug screening</li> </ul>
